The application provides information about the patients health card number and personal data provided through the Municipal Health Department (SMS) registration. During the visit, the health agent will carry out a pre-consultation requesting information related to the patients health. The data is sent to the Ministry of Health and the Department of Health. The saved information remains in the application and can be accessed by health professionals whenever necessary.The application also allows Endemic Disease Control Agents (ACE) to record information to control endemic diseases, during visits to the locations that cover their area of activity, divided into blocks and sides. The agent registers or updates data from the localitys Household and Territorial Registration Form and selects one or more endemic disease forms to fill in, such as: Dengue Form, Chagas Disease, Synanthropic Animals, among others.
